Step 1  
Start by opening the document or selecting an area or a layer you want scanlines on. I will use this girl for this example.
Step 2  
Go to File > New... [CTRL+N].

Set the new document width to 50 and set the units to pixels
Set the new document height to 2 pixels.
Set the new document background to transparent.

Zoom in on the newly created document.

Step 3  
Using a single row selection tool [to get it, click and hold your mouse over the Rectangular Marquee Tool in the tool box], select bottom row of the image. Reset the primary color to black [D]. Using the Bucket Tool [CTRL+G] fill the selection with black color. Deselect [CTRL+D].

Select the whole image by pressing CTRL+A. Now go to File > Edit > Define Pattern. Type in an identifiable name, I used "scanlines". Press OK. Close the temporary image, we wont need it anymore.

Step 4  
Go back to the original document you wanted scanlines in.

I suggest making a separate layer [CTRL+SHIFT+N] for scanlines so that their opacity can be adjusted later on.

With the new layer selected, go to File > Edit > Fill... menu and then select "Pattern" from the drop down option selector. Now find you pattern that you've just made - most likely it is the last one on the list. Select it.

Press OK and viola, you've got yourself the ever so overused trendy scanlines.
